“Delhi
Arogya Kosh” (DAK) is a registered society which provides financial assistance to
the extent of Rs. 5 lacs to the needy eligible patients for treatment of any
illness /disease in Government
Hospital
FINANCIAL
ASSISTANCE FOR TREATMENT IN GOVERNMENT HOSPITALS:
For any illness/treatment/ intervention required by
the patient undergoing treatment in
a Government Hospital run by Delhi Government
or Central Government or Local
Bodies or Autonomous Hospital under State Government.
ELIGIBILITY
- Patient with annual family income upto Rs 3 lacs are
eligible.
- Patient should be a bonafide resident of Delhi for
last 3yrs ( prior to the date of
submission of application)
- Patient requiring treatment for any illness/
treatment/ intervention in a Government Hospital run by Delhi Govt. /Central
Govt. /AIIMS /Autonomous Institutes of the State Govt. /Local Bodies.
Requisite documents for verification of INCOME (any
one of the following):
- Income Certificate issued
from area SDM (or any other officer authorized in this behalf by the Revenue
Department) for income upto Rs. 3 lakh per annum.
Requisite document for verification of DOMICILE for last
03 years (any one of the following):
Note: In case
the patient is a minor, Birth Certificate of the patient and the domicile proof
of either of the parent (any one of the aforementioned document)

HOW TO APPLY
Application
form to be filled by the patient or through his representative alongwith the
following documents in person:
- Income Certificate issued by
the area SDM (for income upto Rs. 3 lakh p.a.) or photocopy of National
Food Security Card (for income less than Rs. 1 lakh p.a.).
- Original Estimate
certificate issued by the treating doctor of the concerned Government Hospital indicating the patient’s disease and the treatment required alongwith
the estimated expenditure of the treatment duly certified by the Medical
Superintendent of the said hospital.
- Two photographs
of the concerned patient, duly attested by the treating doctor of the
concerned Government
- Photocopies
of the treatment record.
PROCESSING OF AN APPLICATION
A COMPLETE application form alongwith all the requisite documents is
processed by the concerned hospital and sent to Director General Health Services
through e-office for his approval.
Thereafter, the application needs approval from the Finance Department,
GNCTD and is thence forwarded to Chairman, DAK for their approvals,
respectively.
Financial Assistance upto Rs. 25000/- is sanctioned at the level of
DGHS, upto Rs. 1.5 lakh is sanctioned at the level of DGHS after approval of
finance department.
Financial Assistance above Rs. 1.5 lakh upto Rs. 5 lakh is sanctioned at
the level of Chairman, DAK
After the due approvals from the competent authority on rotation, the
application comes back to Patient Welfare Cell and sanctioned amount is issued
in favour of the concerned Government Hospital through ECS facility.
The concerned hospital is informed regarding the patient and the sanctioned
amount for further necessary action at the level of hospital.
The applicant, too, is informed through letter sent by Speed-post and,
if contact number is available, telephonically.
Free radiological/Imaging
facilities and free identified surgery for eligible patients undergoing
treatment in hospitals owned by GNCTD:-
The
Governing Body of Delhi Arogya Kosh in its 13th meeting held on
28.02.2017 resolved that financial assistance shall be provided for Ultrasound,
Doppler Studies, Mammography, EEG, EMG, TMT & ECHO from both non-NABH
& NABH ACCREDITED DGEHS EMPANELLED STANDALONE DIAGNOSTIC (Imaging) centres
to any eligible patient who is a bonafide resident of Delhi, identified on the
basis of Voter ID card; Aadhaar Card; Passport; Driving License; Ration Card;
Extract from electoral roll and Birth Certificate alongwith identity proof of
either parent (for children below five years of age), undergoing treatment in
any of the 25 Hospitals owned by GNCTD or in any of the 23 polyclinics attached
to these hospitals or in any of the 05 hospitals run under autonomous mode by
GNCTD.
Financial
assistance shall also be provided for PET and Radio-nucleotide scan from
NABH accredited DGEHS empanelled standalone diagnostic (Imaging) centres to any
eligible patient undergoing treatment in any of the seven hospitals, namely,
GBPH, LNH, GTBH, DDUH, BSAH, JSSH and RGSSH.
In case, an eligible patient undergoing treatment in any other Hospital
owned by GNCTD requires either PET or Radio-nucleotide scan, such patients
shall be referred to any of the aforementioned seven hospitals for further
management and necessary investigation, if required.
The
Governing Body also resolved to amend/change the existing eligibility criteria
of DAK for free identified surgeries in NABH accredited DGEHS empanelled
private hospitals from income upto Rs.3 lakh per annum and bonafide resident of
Delhi for more than three years to any bonafide resident of Delhi, identified
on the basis of either Aadhaar Card or Voter ID or Passport or Driving License or
extract of electoral roll or birth certificate alongwith photo ID proof of
either parent (for children below 5 years), irrespective of his/her income
status. However, the other conditions
for eligibility for referral to be operated in any NABH accredited DGEHS
empanelled private hospital having requisite facilities is only fulfilled if
the eligible patient is undergoing treatment in any Government Hospital owned
and run by GNCTD and is allotted date for surgery beyond one calendar month
after obtaining pre-anesthesia clearance (PAC) by the treating doctor
(surgeon). Eligible patients requiring
CABG shall be referred from GIPMER only after fulfilling the other conditions.
The
Governing Body further resolved that eligible patients suffering from kidney
stone(s) and prostate enlargement requiring surgery can also be referred
after PAC by the general surgeons of secondary and tertiary hospitals, where
such facility is unavailable, hence, there is no waiting list as on date. The reason/justification for referral is to
be recorded by the HOD concerned on the OPD slip and is to be countersigned by
the Medical Director/Superintendent.
